Health professionals are increasingly working with others at a local level to start to take a population health management approach, improve health outcomes and reduce health inequalities.
We have experts who have a track record in supporting different organisations, such as community trusts and PCNs to come together, develop the relationships needed to lead change at place level, with a focus on what will really improve healthcare for local populations. We can work alongside you, transferring knowledge and skills to equip you with techniques to work collaboratively across system partners and leverage your influence both within organisations and across the system.
