As the NHS implements its 10-Year Health Plan, a fundamental transformation is underway in how care is commissioned and delivered. Integrated Care Boards (ICBs) are evolving into strategic commissioners, with a renewed focus on population health, prevention, and value-based outcomes. To realise this vision of strategic commissioning, commissioners, providers and others will need to understand the vision and how they can navigate this change.

In this session we will explore how a population health management framework, combined with patient level linked data, can enable targeted changes to locally delivered integrated models of care.

The agenda covers:

• What is population health management
• How is it different from public health
• Segmentation
• Risk stratification
• Data and analytics
• Cohort specific models of care
• Universal care delivery in a neighbourhood
• Measuring outcomes.

This session will be led by Dr Steve Laitner, GP and Public Health Consultant.

The sessions can be booked as individual courses, or as a whole programme. Where all four courses are booked by the same delegate a discount of £200 or 2 credits will be applied.
