With demand across all health services high making sure that people access the right service, and where appropriate access the support of community or voluntary sector services is important. Social prescribing teams have been supporting people to appropriately access services and helping reduce demand on primary care clinicians.
Where these services are co-ordinated and supported at place or integrated care system level they can be most effective. PCC has reviewed services at a local area level to make the support as effective as possible, using the local resources to best effect.
